Online appointment requests are not confirmed until you receive verification from our office. We will contact you to confirm your preferred time or suggest alternative appointments based on availability.
New patients should arrive 15 minutes prior to their scheduled appointment time to complete the necessary paperwork. Please bring a valid ID, insurance cards, and a list of current medications.
We accept cash, checks, and major credit cards. We also offer financing options, including CareCredit and Cherry, to help make treatment more affordable.
We accept most PPO insurance plans. While we will file insurance claims on your behalf, you are ultimately responsible for paying for the services. Insurance coverage varies by plan, and we recommend verifying your benefits before treatment.
